Cervical spondylosis is a general term for the wear and tear of the spinal discs in the neck as we age. As the discs degenerate, signs of osteoarthritis develop. This can cause achiness and soreness as well as decreased motion in the neck. See Doctor Jo’s blog post about this at: 🤍 First start off with some gentle neck stretches by turning your head from side to side like you are looking over your shoulder. This is neck rotation. Hold it for about 10-15 seconds, and go back and forth about 5 times. Next you are going to take your ear to your shoulder on each side. Make sure you are not shrugging your shoulders up. This is side bending. Hold it for about 10-15 seconds, and go back and forth about 5 times. Now bring your chin to your chest and then looking up towards the ceiling. This is neck flexion and extension. Hold it for about 10-15 seconds, and go back and forth about 5 times. Then you are going to do isometric exercises in each direction. Basically this is when you activate the muscle, but you are not moving it. You can do this by gently pressing into your hand in each direction. Hold each one of these for 3-5 seconds. Now place your hand under your thigh to keep your shoulder down, then side bend your head to the opposite side and gently put pressure with your other hand to get a stretch through your trapezius muscles. Hold these for 30 seconds and perform 3 times on each side. The final stretch is going to be for your levator scapulae muscle. This is the muscle that is used when you shrug your shoulders, and they can get overworked when you are stressed out. Take your arm on the side of the pain and place it above and behind your shoulder. Then take your opposite arm and place on top and slightly behind your head. Look down towards the opposite knee of the pain and apply gentle pressure for a good stretch. Hold these for 30 seconds and perform 3 times on each side. Cristóbal Jacobsen Collado. #valoración #otorrinolaringologia El cuello se explora por inspección, palpación y auscultación. Siempre se explora sentado. La inspección pudiera realizarse de pie, si el sujeto es de menor o igual estatura que el examinador. En su exploración tendremos en cuenta:  Inspección del cuello en su conjunto.  Exploración de la glándula tiroides.  Examen de los ganglios linfáticos.  Examen de los vasos del cuello.  Exploración del resto de las estructuras. INSPECCIÓN DEL CUELLO EN SU CONJUNTO. En la inspección del cuello en su conjunto debemos explorar su forma (corto, largo, normal), volumen (ancho, delgado, normal), posición (central, con desviación lateral, en flexión, en extensión), su movilidad, latidos, y la presencia o no de tumoraciones. La exploración de las estructuras musculosqueléticas y la movilidad del cuello serán descritas en esta Sección, cuando abordemos el examen del sistema osteomioarticular, y los latidos, en el capítulo del examen del sistema vascular periférico. Todas las alteraciones patológicas del examen físico, serán descritas en la Sección II. Además de las regiones del cuello propiamente dichas, deben explorarse las regiones parotídeas, submaxilares y sublinguales, así como la región supraclavicular y la nuca. EXPLORACIÓN DE LA GLÁNDULA TIROIDES La glándula tiroides está situada en la región anterior del cuello (en la unión de los dos tercios superiores con el tercio inferior); consta de dos lóbulos laterales unidos en su base por un istmo, que le confiere una forma de “U” o de mariposa; y tiene un peso aproximado de 20-30 g. El lóbulo derecho es ligeramente mayor que el izquierdo. Cada lóbulo tiene unos 5 cm de largo y 2 cm de ancho. El examen físico del tiroides se realiza mediante inspección y palpación. Normalmente, el tiroides no se aprecia en la inspección y prácticamente, no se palpa; si es de tamaño normal, solo se logra su palpación con especial cuidado y con reglas precisas. Si con ello se detecta aumento de volumen, debe realizarse también la auscultación de la glándula (presencia o ausencia de soplo en tiroides vascularizadas) y la medición del cuello. La glándula está unida a la tráquea y se eleva cuando el individuo traga. Para que la inspección y la palpación sean de mayor utilidad, usted debe explorarla también, mientras el sujeto traga; tenga preparado un vaso de agua, para que la persona trague sorbos, cuando se lo indique. Es importante tener presente que la glándula tiroides puede estar aumentada, aunque dentro de los límites normales, por distintas circunstancias: país, altitud, pubertad, embarazo, menstruación, etc.; es decir, en los estados fisiológicos de alarma de cualquier orden, en la que puede aumentar de volumen y tornarse más activa fisiológicamente. Inspección Observe la región anterior del cuello de frente y de perfil, de ser posible con una iluminación tangencial, que puede ayudar a detectar mejor, cambios sutiles en el contorno o la simetría. Normalmente solo puede verse el istmo glandular, sobre todo al tragar, con mayor frecuencia en mujeres jóvenes. Primero, pida a la persona que mantenga la cabeza y el cuello en una posición normal y relajada. Observe si existe alguna desviación de la tráquea, así como las delimitaciones del cartílago tiroides y cricoides, y fíjese si hay algún aumento de volumen. Después, pida que extienda ligeramente el cuello, inclinando la cabeza hacia atrás, y que trague un sorbo de agua. Observe en ese momento, el movimiento simétrico hacia arriba de la tráquea y los cartílagos laríngeos y, de existir algún aumento de volumen, si este también se desplaza.

Doctors of Physical Therapy can effectively and safely perform spinal manipulations just like chiropractors. Grade 5 low amplitude high velocity manipulations can be extremely effective in certain subgroups of individuals. They can improve range of motion, reduce pain in various ways, decrease muscle tone/guarding and improve muscle activation/recruitment. However, the exact mechanisms of how spinal manipulation truly works is still debated. But, we can say that is it more than just "realigning the spine", and my thoughts are more neurophysiological in the sense that we are providing a novel stimulus to the system which enacts the changes/benefits we see following manipulation. Research studies DO show that there are changes in the central nervous system (brain regions) immediately following spinal manipulation. I wanted to share an exert from our professor, Dr. Rob Landel PT, DPT, OCS, FAPTA, regarding physical therapists performing manipulation. "The safe application of any technique requires the use of clinical judgment, specifically, determining not only who is appropriate for the technique, but also for whom the technique is contraindicated. Adequate clinical judgment is based on knowledge of anatomy, pathoanatomy, biomechanics, pathomechanics, pathology, and a differential diagnostic process. Also, the decision of whether or not to perform a manipulation must be evidence-based, i.e. taken into account the best available evidence, the clinician’s experience, and the patient’s needs and goals. As I noted in my last post, all of these aspects are basic components of any Doctor of Physical Therapy program. Furthermore, this knowledge and clinical judgment is tested extensively through written, oral, and practical examinations, as well as put into practice during as much as one year of supervised clinical experiences. Finally, in order to become licensed in the state of California, every physical therapist must pass their licensure exam, which includes testing their knowledge and clinical judgment on manipulation, as required by the Federation of State Boards of Physical Therapy. Thus it is clear that physical therapists are well prepared to safely perform manipulation techniques, and in fact are required to be safe in their application before being allowed to practice." Cervical Cancer Signs & Symptoms (& Why They Occur) Cervical Cancer (cancer of the cervix) is the 3rd or 4th most common gynecological cancer worldwide with an estimated age of onset of 52 years old. Cervical Cancer is caused by human papillomavirus virus (HPV) infections, with particular types of HPV being more problematic than other types. Cervical Cancer is often discovered in the early stages of disease by screening and diagnostic methods (pap smear and colposcopy), so in some cases patients may be asymptomatic. However, as the disease progresses, more signs and symptoms of Cervical Cancer become apparent, including abnormal vaginal bleeding and other findings related to metastasis of the cancer. Spinal disc degeneration, also known as degenerative disc disease, is not actually a disease. Rather, it refers to changes in the spinal discs as a result of chronic wear and tear, injury, or natural aging that occurs over a long period of time. These changes include loss of fluid in the discs (a condition called disc desiccation), tears of the disc annulus (the outer layer of the disc), and growth of nerve fibers in the damaged discs. Degenerative disc disease occurs in the neck or lower back regions of the spine. This happens because these regions are under more pressure than others. Discs in these regions have more movement, and therefore wear and tear much more. So, the intervertebral discs are located between the vertebrae of the spine and serve as padding between them. These discs help with the stress that occurs in the spine with movement. With age or due to some other reasons like repetitive movements, the elastic part of the disc can wear and tear. There are several reasons for this, including loss of fluid, which is one of the main components of the discs, or some changes in disc structure, such as cracks or tears. Cervical Cancer | Risk Factors, Pathophysiology, Symptoms, Staging, Diagnosis, Treatment & Prevention Cervical Cancer is a cancer of the cervix, which is the lower end of the uterus. Cervical cancer is caused by infection with the human papillomavirus (HPV), with certain types of HPV (types 16, 18) having a higher risk of causing cervical dysplasia and cervical cancer. Cervical cancer results after repeat or long-standing infections with HPV. A variety of factors increase the risk of Cervical cancer including immunosuppression and smoking. In some cases Cervical cancer can be asymptomatic, although as the condition worsens, cervical cancer will cause signs and symptoms including vaginal discharge and vaginal discomfort among other symptoms. In this lesson, we discuss all of the risk factors for getting cervical cancer, the signs and symptoms including symptoms of metastasis of the cancer, staging of Cervical cancer by pap smear and colposcopy, how it’s diagnosed and how it’s treated. NOTE: In addition to what was mentioned in this lesson with regards to diagnostic procedures, imaging can be used to help stage the cervical cancer (especially in Stage IV). As mentioned, there are seven individual bones; the first, second and seventh are uniquely shaped, whereas the third to the sixth are relatively similar in form. C1, otherwise known as the atlas, is the first cervical vertebrae as well as the first vertebrae in the spinal column. It supports the skull which sits directly above it and it only has an anterior arch and a posterior arch, with no body or spinous process. The axis or C2 as it is clinically called, has its odontoid process (dens) located on its superior surface. It has a large bifid spinous process and in contrast to C1, a small transverse process which houses its foramen transversarium. The third through sixth cervical vertebrae have relatively small vertebral bodies with posterior and lateral pedicles. The short spinous processes are bifid in shape while the vertebral foramina are somewhat triangular in shape. Within each transverse process there is a transverse foramen/foramen transversarium, which houses the vertebral artery from the level of C6 upwards. Spinal disc degeneration, also known as degenerative disc disease, is not actually a disease. Rather, it refers to changes in the spinal discs as a result of chronic wear and tear, injury, or natural aging that occurs over a long period of time. These changes include loss of fluid in the discs (a condition called disc desiccation), tears of the disc annulus (the outer layer of the disc), and growth of nerve fibers in the damaged discs. Degenerative disc disease occurs in the neck or lower back regions of the spine. This happens because these regions are under more pressure than others. Discs in these regions have more movement, and therefore wear and tear much more. So, the intervertebral discs are located between the vertebrae of the spine and serve as padding between them. These discs help with the stress that occurs in the spine with movement. With age or due to some other reasons like repetitive movements, the elastic part of the disc can wear and tear. There are several reasons for this, including loss of fluid, which is one of the main components of the discs, or some changes in disc structure, such as cracks or tears.
